The International Day of People with Disability Mayoral Awards honour and recognise the outstanding achievements of people with disability and individuals and organisations within the City of Playford that have improved the lives of people with disability and grow the region as a truly inclusive community.

In 2016, the Mayoral Awards will be announced at the International Day of Disability - Celebration of Ability event held on Friday, December 2.

Those nominated for a Mayoral Award will have made an ongoing and significant contribution to the Playford region to improve the quality of people’s lives. The nominee/s can be an individual, an agency or a group.

All nominations will be considered by the Mayor and the Quality Coordinator Disability and Access Inclusion.

The Mayor may require additional information in making the final determinations, so please provide the names and contact details of two referees who can attest to the statements made by the Nominator.

The nomination form and accompanying documents should be in neat handwriting or typed and the submission will not exceed six pages.

Applications will close Sunday, October 31.

Person/Organisation who have made a significant contribution to people with disability during the current year and/or given outstanding and continuous service over a number of years. This contribution can be by means of design or inclusion practices.
